How Do I Qualify for Coverage and Maintain My Coverage?


You become eligible for coverage on the first day of the first month following a period (12 months or less) during which you have worked at least 1,000 hours (350 hours in the last 3 months) with a Contributing Employer. With respect to your life insurance benefit, you will become eligible on the first day of the month following the month in which you are hired.

 

Once you are enrolled and receiving benefits, your coverage will continue:

1. for each month you are working, if you have at least 350 credited hours of work in covered employment in the preceding 3 consecutive month period or 1,200 credited hours of work in covered employment in the preceding 12 consecutive month period; or

2. for each month you are laid-off or disabled, provided you have 1,200 credited hours of work in covered employment in the preceding 12 consecutive month period or 2,400 credited hours of work in covered employment in the preceding 24 consecutive month period, or 3,600 credited hours of work in covered employment in the proceeding 36 consecutive month period.
